Fertility as an ability gifted by God

What is fertility?

Fertility,as a capacity to reproduce, lets the fertile men to be able to become father and fertile women to get pregnant and carry their baby to full term, with a live birth nine months after conception. This all happens naturally as a result of sexual intercourse.

​Somewhere around 10% to 15 % of couples around the world want to start a family but face problems with their fertility. This problem may be occurred due to the problem with male partner or female partner, or with both of them. Fertility clinic at in India as well as in Thailand offers a wide variety of infertility treatments to let the couples experiencing infertility issues to become fertile. As much as we grow up, our fertility starts to decline slowly, especially in women. The possibility of pregnancy by natural intercourse and even after infertility treatments drops severely between the ages of 35 and 40 years. Most of the time, issues with fertility can be a problem at our late teens and early 20 because at this age, our desires for sex reach higher and our fertility level also comes at its peak. This combination leads to a high level of unwanted teenage pregnancies in many countries in the western world.

Female fertility:

Women's fertility hits the highest point in the early age of 20s and then drops considerably after the age 35. Menopause typically occurs during a women's midlife (usually between ages 45 and 55). During menopause, hormonal production by the ovaries is reduced, eventually causing a permanent cessation of the primary function of the ovaries, particularly the creation of the uterine lining (period). This is considered the end of the fertile phase of a woman's life. According to an epidemiologist, there are some statics showing the conditions for probability of women’s pregnancy naturally without using fertility drugs or in vitro fertilization.

Male fertility:

As much as age increased in male, the fertility problems in male are found in the form of decline in semen volume, sperm motility, and sperm morphology. Sperm count declines with age, with men aged 50–80 years producing sperm at an average rate of 75% in comparison to the men aged of 20–50 years. However, an even larger difference is seen of fertility in men. This difference of fertility occurs due the lack of formation of mature sperms in seminiferous tubules in the testes.
·        90% of the somniferous tubules restrain mature sperm in males aged between 20 to 39 years
·        50% of the somniferous tubules restrain mature sperm in males aged between 40 to 69 years

·        10% of the somniferous tubules restrain mature sperm in males aged of 80 years or above.

Preliminary treatment to fertility problems:

1.      Treatment for the woman:
There may be lots of reasons for thefertility problems in women even sometimes the reasons and the sources or fertility problems are out of the reach of treatments. The following known reasons are;
Problems with ovulating. Treatment may include taking medicines, such as: i)        Clomiphene. It stimulates your ovaries to release eggs properly.
ii)      Metformin. It's used to treat polycystic ovary syndrome.
Unexplained infertility. If the source and the reasons are not known in you and your partner to reproduce the baby naturally then these treatments may be appropriate; i)        Clomiphene
ii)      Hormone injections
iii)     Insemination.
Blocked or damaged tubes. For the problem of blockage in fallopian tubes, tubal surgery is recommended to the patient.

Endometriosis. If mild to moderate endometriosis seems to be the main reason for your infertility, treatment may include laparoscopic surgery to remove endometrial tissue growth. 2.      Treatment for the man: There are not much problems with man’s fertility in comparison to woman. Initially the process of insemination is recommended. Fertility Techniques:

Artificial Insemination

The procedure of artificial Insemination emulates the natural reproduction process inside the uterus, turning the process easier for the sperm to reach the right place at the time of the ovulation. Artificial Insemination is brought to be used when sperm have problems reaching the uterus properly. This may be caused by blockage in fallopian tube or due lack in the quantity of low sperms. At, Artificial Insemination is done with the partner’s sperm, it is known as artificial Insemination with partner sperm. When we cannot get sufficient sperms or healthy sperms from the partner or in the case when there is no male partner, then we look for a sperm donor. Thus we call this as an artificial Insemination with donor sperm. This technique is relatively straight forward and gives very good results. This procedure is performed in two ways
·        Artificial Insemination with partner sperm ·        Artificial Insemination with donor sperm.

Though, the issues are not solved out successfully every time and this leads the patients to have In Vitro Fertilization(IVF).

In Vitro Fertilization (IVF): Fertilizing an egg with a sperm outside the uterus in the laboratory is performed in the procedure of In Vitro Fertilization treatment at If there is issue with sperm quality such as sperm does not reach to the appropriate destination then a sperm is injected into each egg to fertilize it using an extremely fine needle and this process of injection is called Intracytoplasmic Sperm Injection (ICSI). The situation when fertilization takes place productively, the egg becomes a pre-embryo and is placed in the uterus to continue for developing maturely. After having the process of fertilization properly, the most of best and healthier embryo is taken for further procedure, and this procedure is known as Intracytoplasmic Morphologically-selected Sperm Injection (IMSI). For In Vitro fertilization, sperm can be provided either by the partner or by an unidentified donor. It may be that the woman is not producing eggs or that her eggs are not fit for use, in this case, eggs of an anonymous donor can be used. This procedure of IVF can be performed in different ways are following;
·        In Vitro Fertilization with own eggs and partner’s sperm
·        In Vitro Fertilization with donor eggs and partner’s sperm
·        In Vitro Fertilization with own eggs and donor sperm,
·        In Vitro Fertilization with donor eggs and donor sperm.
·     Medication choices

For male:

i)        Gonadotropin- It increases the secretionof hormones which are needed for sperm production.

ii)      Bromocriptine and cabergoline decrease prolactin levels. The excess secretion of prolactin can affect badly on releasing of testosterone as well as on production of sperm.

For female:
i)        Gonadotropins- These hormone shots stimulate the ovaries to produce mature eggs. ii)      Medicines for polycystic ovarysyndrome (PCOS). If you're not ovulating because of PCOS, your doctor might suggest that you take a drug such as metforminalong with clomiphene. iii)     Bromocriptine and cabergoline decrease prolactin levels. High levels of prolactin can cause to the process of ovulation.
